个人认为选上述最后一个。因为确实提到At the bottom of, 但是说的应该不是High Road，是另一条路，In the middle of High Road之后会发现左边有一条路，Health Centre is at the bottom of that road.
5.申请表。中心会给这位女士邮寄申请表。问申请表怎样送回health centre.
选项里面有邮寄、亲自送回中心以及附上支票（with a cheque）送回。
应该选亲自送回中心 （5题）就可以了。
因为提到的check不是支票，是说要送回中心给前台核对（check）一下。
然后是个人信息表格题。
